The OSHP Preceptor of the Year Award

Nomination Form I Previous Recipients

Purpose

To recognize a pharmacist who has demonstrated excellence in precepting pharmacy students and/or pharmacy residents.

Description

The recipient of this award will have demonstrated significant and continued dedication to the education of pharmacy students and/or pharmacy residents above and beyond the general expectations of a pharmacy preceptor.


Criteria

The nominee must be a pharmacist practicing in the state of Ohio who is currently or has previously served (within the past year) as a preceptor to pharmacy students and/or residents in the state of Ohio. The ideal nominee should have served as a preceptor for a minimum of one year and should demonstrate the following:

      • Exemplary preceptorship ability
      • Interprofessional Engagement and advancement of Interprofessional Education
      • Promotion of innovative scholarly and/or practice activities
      • Advocacy for improvement of training opportunities

Nomination

Nominations may be submitted by any OSHP member, OSHP Board of Directors, and Local Affiliated Chapters of OSHP. A CV is required for a complete application submission.

Selection

Made by OSHP Division of Professional Affairs; final approval by the OSHP Board of Directors.
