The OSHP PGY2 Resident Award
Nomination Form
Purpose
To recognize a pharmacy resident who has demonstrated exceptional skills in the area of residency practice.
Criteria
The nominee must be a current PGY2 resident. Residents enrolled in a dual PGY1/PGY2 program, can be considered for this award during their PGY2 year. Board members are not eligible for awards during their term of service to OSHP.
May be submitted by any member of OSHP, OSHP Board of Directors, or Local Affiliated Chapters. A letter of recommendation from a pharmacy residency director or a program preceptor must accompany the nomination (maximum three letters).
Selection
Made by OSHP Division of Professional Affairs; final approval by the OSHP Board of Directors.
