Purpose: Fellowship in the Ohio Society of Health-System Pharmacists (OSHP) recognizes excellence in health-system pharmacy. Fellowship is granted to pharmacists who have distinguished themselves within OSHP and have made significant and sustained contributions to OSHP and the profession of pharmacy in accordance with the mission to optimize patient health by advocating for the advancement of pharmacy practice to promote comprehensive, quality care across the health-care continuum. Fellowship status is for life as long as the individual is an active member of OSHP.

Selection: Applicants must meet minimum criteria for at least 3 of the 5 categories indicated under the “Sustained Contribution to OSHP” category as below. Additionally, applicants must earn a minimum of 25 points coming from the “Sustained Contributions to the Profession of Pharmacy” category. Final decisions for fellow status are at the discretion of the committee.

Procedure: Candidates who meet eligibility criteria are encouraged to submit their application and their CV. The applicant’s CV will be used to verify the activities noted in the application. Three letters of support from pharmacists who can attest to the applicant’s qualifications must be submitted with the candidate’s application. These letters of support must address the applicant’s OSHP involvement, advancement of pharmacy practice, scholarly activity, and teaching/contribution to the future of pharmacy. There is no fee to submit the application for OSHP fellowship.

Award: Fellows will receive the designation of Fellow of OSHP (FOhSHP)

Exclusions: Contributions evaluated must have been completed as a pharmacist and not as a student. Current OSHP board members and employees of OSHP are not eligible to apply during their terms in office.
