Purpose

To recognize a pharmacy technician of high integrity who has made significant contributions toward supporting the practice of pharmacy within the health-system organization.

Criteria

The focus of this award is based upon recent accomplishments within the health-system organization.

Nominations

May be submitted along with supporting materials(s) by any non-OSHP individuals, any OSHP member, OSHP Board of Directors, or Local Affiliated Chapters of OSHP.

A letter of recommendation from the immediate supervisor must accompany nomination.

Selection

Made by OSHP Division of Professional Affairs; final approval by the OSHP Board of Directors.
