OSHP Residency Program Director Award

Nomination Form I Previous Recipients

Purpose

To recognize a pharmacist who has demonstrated excellence in the position of Residency Program Director.

Criteria

The nominee must be a pharmacist practicing in the state of Ohio who is currently or has previously served (within the past year) as a residency program director in the state of Ohio. The ideal nominee should demonstrate:

Dedication to teaching and learning the practice of Pharmacy

Exemplary preceptorship ability

Professionalism

Having established/expanded residency program positions

Promotes and advocates for residency program training in the state of Ohio

Nomination

May be submitted by any OSHP member, OSHP Board of Directors, or Local Affiliated Chapters of OSHP. The nominee’s affiliation and involvement with residency training should be well described in a letter of recommendation. Additionally, characteristics and a history of the residency program the individual is involved with should be provided. A CV is highly recommended for submission as well.

Selection

Made by OSHP Division of Professional Affairs; final approval by the OSHP Board of Directors.
